To refer a patient, please complete the referral form and submit it along with the patient’s medical records. As a minimum, recent bloodwork and a urinalysis performed within the previous six months are required.
For Pet Owners
If you are interested in a nutrition consultation for your pet, please ask your veterinarian to complete the referral form. Once the referral has been received, we will contact you directly by email with the next steps.
What to Expect
Nutrition consultations focus exclusively on nutritional assessment and recommendations. Consultations are available for both healthy pets and pets with medical conditions that may benefit from nutritional management.
During the consultation, Dr. Grant will:
- Review your pet’s medical history and current diet
- Assess nutritional needs based on your pet’s age, lifestyle, and health status
- Answer nutrition-related questions
- Recommend an appropriate commercial diet and feeding plan, or
- Formulate a customized homemade diet recipe when indicated
A written consultation summary, including all recommendations and any formulated recipes, will be provided to the referring veterinarian within one week of the appointment. The referring veterinarian remains responsible for ongoing case management and follow-up care.

Referral Process
Step 1:
The referring veterinarian submits the referral form, medical records, recent bloodwork, and urinalysis.
Step 2:
Once the referral is received, the pet owner will be contacted and provided with a diet history questionnaire.
Step 3:
After the completed diet history has been reviewed, a consultation appointment will be offered. A deposit equal to the consultation fee is required to reserve the appointment. Fees for homemade diet formulation are due at the time of the consultation.
Step 4:
The consultation takes place at Red Tree Animal Clinic.
Step 5:
A consultation summary, including recommendations and any formulated homemade diet recipe, is sent to the referring veterinarian within one week of the appointment.
Please note: Each consultation is specific to a single pet. Separate referrals and appointments are required for multiple pets, even if they live in the same household.
